BEHAVIOURAL AND EMOTIONAL PROBLEMS IN SCHOOL GOING ADOLESCENTS 2

Background

Adolescence is marked by immense turmoil in emotional and behavioural spheres.1 WHO defines adolescence as the period of life between the ages of 10-to 19 years.1The adolescent struggles to develop his individuality while still conforming to societal norms2. Rapid urbanization and modernization have exposed them to changes in society. The resultant breakdown in family structure, excessive or minimal control confuses the adolescent and makes him/her especially vulnerable to maladaptive patterns of thinking and behavior3. Healthy adulthood depends upon successful resolution of these emotional and behavioural problems.4 Treading on this tightrope, most adolescents go through to adulthood normally. All adolescents may not be so fortunate, to get the ideal societal support for this smooth transition. Some develop maladaptive patterns in emotional and behavioural spheres .This augers ill for the individual's future resulting in depression, delinquency and suicides among other problems.5

Of late there has been a rise in the prevalence of mental illness and maladaptive behaviours among adolescents4. WHO estimate shows that up to 20% adolescent have one or more mental or behavioural problems4. Studies conducted in different parts of the world show that prevalence of behavioural and emotional problems in adolescents ranges from 16.5%5 to 40.8%6 and in India it is in the range of 13.7%7 to 50%8. As adolescents form one fifth of India's population9, this means a sizable disease burden on the society. Lack of data on the subject precludes an assessment of the magnitude of the problem which is essential for effective health care planning. This study has been planned to assess prevalence and pattern of maladaptive behavioural and emotional problems among school going adolescents. The association between socio-environmental factors and the emotional and behavioural problems in this age group was also studied.

Method

The present study was conducted in Chandigarh, Union territory. Only high schools and higher secondary schools were taken for the study. As per data collected from office of director of education, Chandigarh, the Chandigarh municipal area has 130 schools in the category of high schools and higher secondary schools catering to a total of 23696 students. To ascertain similar environment for boys and girls, only co-educational schools (n=114) were takenup for the study and boys-only or girls- only schools were excluded (n=16). Among co-educational schools, 87 were government and 27 were private schools with a total number of 16700(70%) and 6996(30%) students respectively. Students of government and private schools represent different socioeconomic strata due to high cost of education in private schools. Hence, students from both types of schools were included in the study.

Literature review indicated that the prevalence rate of emotional and behavioural problems among adolescents in India ranges from 13.7% to 50% 7. Since no such figure is available for Chandigarh, the required sample size was calculated presuming prevalence of 27.28% (median of reported prevalence worldwide). At this level of disease prevalence, the required sample size came out to be 1088. Assuming a non response rate of 10% it was planned to include a sample of 1200 students.

The study population was selected by stratified random sampling. The number of students in government and private strata was selected by probability proportionate to size (PPS) sampling technique. 850 students from government schools and 350 students from private schools were taken for the study. A list of all the government and private co- educational school was prepared and 7 government and 3 private schools were randomly selected to get the required sample size. An average of 120 students was taken from each of the selected schools. For equal representation of all the classes from 7th to 12th, 20 students were taken from each class. To meet the required sample size only one section of each class was selected for the study. These students were selected by simple random sampling.

Behavioural and emotional problems were assessed by the Youth Self-Report TM (2001) developed by T.M. Achenbach and L.A. Rescorla10. It is a questionnaire used widely to screen adolescents with psychiatric problems. The content, criteria and construct validity of this questionnaire has been supported by four decades of research consultation, feedback and revision, as well as by findings that all items discriminated significantly (P< .01) between demographically matched referred and nonreferred children10. In various studies conducted in world, the sensitivity and specificity of this questionnaire has been found to vary from 85%-100% and 30%-40% respectively11. This questionnaire is valid for ages 11-18 years. It is based on the widely used Rutter&s Child Behaviour Checklist. Although it is a part of multiaxial assessment of psychological functioning of an adolescent, it can be used to provide data on adolescent&s emotional and behavioural problems. It contains competence scale score and problem scale score. For this study only problem scale score was taken.

The YSR identifies emotional/behavioural problems in three groups (comprising of subgroups)–internalizing (withdrawn depressed, anxious depressed and somatic complaints) externalizing (rule breaking behaviours and aggressive behaviours) and neither internalizing nor externalizing (social problems, thought problems and attention problems). The Problems identified by YSR have been found to be very consistent with DSM-IV diagnostic categories10.

The YSR was translated by a team of professionals from English to Hindi and then retranslated from Hindi to English to note any deviation in meaning. After serial modifications in language and review, the research team arrived at a questionnaire which retained the exact sequence, content, construct and the import of the original question as far as possible rather than a verbatim translation. Thus, the pro forma was available in English as well as Hindi. Students were allowed to use either of the two as per their convenience. Pretest of the questionnaire was done in classes V to XII assuming that the students of these classes are in adolescent age group. It was observed that the students below class VII had left most of the item in the questionnaire even after best efforts on the part of the researcher. Thus, based on pretest results and the difficulties reported by students and researchers in filling the form and interoperating the responses it was decided to administer this proforma in class VII and above. The students of these classes were in the age group of 12 to 18 years old so this group was included for the study. The study was conducted after obtaining written permission from Director of Education, Chandigarh and the principals of the ten schools selected for the study.

Approval of ethical committee of the institute was sought before conducting the study. Informed and written consent was obtained from parents on a printed pro forma distributed a day prior to filling the questionnaire. On the day of administering the questionnaires, children were addressed by their teachers followed by the research team and were offered to participate or opt out of the study. The questionnaires were anonymous and had no mark of identity. The completed Youth Self Report questionnaires (112 questions) were scored according to the Manual for the ASEBA School-Age Forms and Profiles (2001) to identify emotional and behavioural problems. The scores (0 for not true, 1 for sometimes true and 2 for always true), for each item in the questionnaire were entered in EXCEL sheet (MS Office 2003)

For assessing the family environment, an indigenous 23 items questionnaire was developed by a senior adolescent psychiatrist (co-author). Some items comprised factual information about family-size, type, income, educational background, employment, chronic illness in adolescents and addiction in family. Other questions were designed to assess child's self esteem, satisfaction with his academic performance, parents' satisfaction with child's academic performance and child's perception of closeness and proximity to one of the parents. There were separate items for assessing child abuse. This questionnaire was also written in both languages, pretested and suitably modified before forming a part of the questionnaire set.

All statistical analysis was done using SPSS for WINDOWSTM, version 16.0. To assess the association of socio-environmental factors with psychiatric morbidity, chi-square test and multiple logistic regressions were carried out.

Results

The study was conducted from January 2008 to July 2008. 1150 school- going adolescents in 12 to 18 year age group participated in the study. 50 students refused to participate. Responses of 27 students were excluded due to omission of more than 8 problem item in YSR. Thus, 1123 respondents (516 girls and 607 boys) were the subjects for the study. The study shows that the overall prevalence of behavioural and emotional problems across age and sex categories was 30.4%. There was a significant increasing trend in prevalence from onset of adolescence till 17 years of age after which a decline was noted. (Table 1)

In the present study, a higher prevalence of behavioural/ emotional problems (33.7%) was observed in adolescent girls as compared to boys (27.5%). Also, whereas prevalence in boys shows a peak around 14-15 years followed by a steady decline to 26.3% by 18-19 years of age, girls show a continuous rise in psychiatric problems with age, 43% girls having problems by 18-19 years.

On analysis of pattern of emotional and behavioural problems in adolescents, it was found that internalizing syndrome (28.6%) was the most common problem amongst them followed by the neither internalizing nor externalizing (19.5%) group. In internalizing group, most (22.08%) of the students were noted to be anxious/ depressed. Social problem was observed to be the most frequent (9.3%) among neither internalizing nor externalizing group. Aggressive behaviour was the commonest (11.8%) among externalizing group. (Table II) Internalizing problems mainly anxiety and depression were commoner in females than males. 24.8% females were anxious / depressed compared to 19.7% males. More boys (18.4%) than girls (12%) were found to suffer from externalizing disorders. 13.3% males had aggressive behavioural problem as compared to 9.7 % females.

Problem of rule braking or delinquency was twice more in boys as compared to girls. On assessing the association of various socio-environmental factor and emotional and behavioural problem among adolescents it was observed that the children of parents with addiction of alcoholism or illicit drugs, developed up to three times as many behavioural and emotional problems as compared to children of parents without addiction. Parents who were consuming six or more standard drinks in one sitting at least once a week were considered as addicted to alcohol.12

These problems were more (41%) among physically abused adolescents. Chronic illness in adolescents was also identified to be significantly related to problem under study. Although only 3% students in our study had chronic/debilitating illness but amongst them 73.5% suffered from the psychiatric problem as compared to their healthy counterparts (31%).

The questionnaire tested the student's perception of his academic performances as well as their parents' perception about the same. 75% of the adolescents who thought they were academically poor were found to be suffering from behavioural/emotional problems. 43% of the adolescents who thought that their parents were unhappy with their studies had developed the above mental health problem. On univariate and multivariate analysis, both of these factors were found to be significantly related to behavioural and emotional problems (p< 0.001). Significantly more (34.4%) adolescents belonging to families with marital discord were found to be suffering from behavioural and emotional problems as compared to adolescents of families with good inter-parent relationship.

Adolescents who perceived that they are not loved enough by their mothers showed behavioural and emotional problems twice more than those who felt loved by their mothers. This difference was found to be highly significant both on univariate and multivariate analysis. (p< 0.001).Adolescents with low self esteem (30.7%) as judged from their opinion about their appearance, academic performance and overall opinion about self suffered more(65%) behavioural and emotional problems as compared to those who did not report feelings of low self esteem(14%).

Factors like type of school, type of family, socio-economic status, mother&s employment status, presence of chronic illness in family members, educational status of mother and relationship with father were not found to be significantly associated with behavioural and emotional problems.

The final model of Multiple Logistic Regression analysis showed seven important independent factors which have strong association with behavioural and emotional problems. Relationship with mother had the highest odds (3.489) followed by addiction in father (2.642) and interparent relationship (1.402). Discussion

In the present study, the prevalence of behavioural and emotional problems among 12 to 18 years school- going adolescents was found to be 30.4%. Robert&s et al in a meta-analysis of 52 studies done in 20 countries of the world, found that prevalence of psychopathology among adolescents (12 to 18 years) varies from 6% to 41% 6. In a study on school-going adolescents of Delhi, 50% of the students were found to have problems of emotional maladjustment9. Similar study done in adolescents of Bangalore city reported that 20 % of the children had psychiatric problems13. Thus a third of our adolescents, by even the most conservative estimate, are suffering silently without even being recognized, except for some localized studies.

The morbidity pattern was found to be different in males and females. The prevalence in boys dropped after 17 years but it continued to rise among girls. The rise in emotional and behavioural problems from 12 years up to 17 years is reported in previous studies7 . Further rise in prevalence of behavioural and emotional problems with age among girls, who are known to have puberty an average of 2 years before boys hence have more years of adjustment to somatic problems, is intriguing. It seems that gender biases and imposition of restrictions on Indian girls have led to rise in psychiatric problems with age.

We report here that 75% of students feeling that they lag behind in their studies (23% of study subjects), have emotional and behavioural problems. Other authors have also reported that achievement pressure was an important factor in causation of psychiatric problems14-19

An interesting finding in our study is the relation of adolescents' bonding with his mother and prevalence of behavioural and emotional problems. This applied to both sexes of adolescents. Adolescents' perception of a secure bonding with the mother was found to be significantly associated with the problem under study but no significant difference in prevalence of the same was found among boys and girls when the bonding with mother was insecure. We can therefore infer that a loving family with marital harmony protects against mental ill health. However relationship between father and adolescent was not found to be significantly associated with the problem under study. Various other researchers have also emphasized the importance of the adolescents' relationship with mother. Palosaari et al16 found that girls who had good bonding with their mothers had high selfesteem and therefore lesser psychiatric problems. Lack of ability to talk to the mother, has been shown to be significantly associated with development of depression in children of 12 to 17 years of age18, 19. This affects the female child more than male child.

In the present study, parental discord was found to be related with emotional and behavioural problem among adolescent. Inter-parent conflict has been reported to be a factor significantly related to suicide among adolescents by many authors17-20. They have also found that 16-year-old adolescents of divorced parents had more somatic complaints and lower self-esteem than children of intact families.18

Alcoholism was found to be the single most common addiction and a three times higher prevalence of behavioural and emotional problems in adolescents from families with addiction. Youngsters whose parents abused alcohol probably didn&t rely on their parents; they took more support from friends and siblings. Similar findings have been observed by other authors. 15.

Nearly twice the number of adolescents who gave history of physical abuse in our study suffered from behavioural and emotional problems. Many authors have closely related physical abuse or punishment to development of psychiatric morbidity18 Abused children were seven times more likely to develop a major depressive disorder. Abused boys were shown to have equal chances of developing depressive disorder as the abused girls.

Our study has a few important limitations. Multi informant studies comprising of responses of parents (Child Behaviour Checklist), teachers (Teacher Report Form), and the adolescent himself (YSR), with cross informant comparisons are ideal for screening for mental health of the propositus11 followed by psychiatric clinical evaluation3 for a definitive diagnosis. We have used only a single informant report form for our study. Keeping Indian scenario in mind, the above decision was taken. In government schools, one teacher caters to large number of students with an equally cramped academic curriculum to cover; there is little possibility of knowledge of family, emotional make-up, and behavioural details of each student. Also there is little likelihood of compliance if a teacher is made to answer 100 odd questions about 40 or more students; the truthfulness and usefulness of such a work would also be suspect. With variable educational levels of parents, it would again preclude the use of written questionnaires being filled by parents.

We did not subject adolescent to a detailed psychiatric assessment following positive result in questionnaire based psychiatric assessment. Psychiatric referral of school students involves parental communication, consent and costs apart from burden on outpatient psychiatric services. For confidentiality, none of the questionnaire wore any marks of identification so individual identification or communication of result to teachers or parents could not be done.

Adolescence is characterized by self doubt, concealment and modification of fact which is known limitation of all self reported questionnaire based studies. Questionnaire still form any important part of population based study and is a valuable screening tool.

Conclusion

A sizeable population of our adolescents need support in coping with emotional and behavioural problems. Though many children of families with problems may be normal, knowledge of the family environment and problems in the adolescents-identifies the adolescent-family dyad that may need attention. It indicates towards the need for a multipronged intervention to prevent these problems in adolescents. School based mental health services can handle the problem in most effective way by providing help to the sufferers at earliest. It also seems imperative to have a post of counsellor in every school. The academic achievement and assistance for the same may also be required as that is found to be an important determinant. A community intervention for addiction may also be required and school can become the base using innovative programs like student drama club, street plays etc. thus educating the family as well as the school children against addiction too.

Method

 

A school based cross-sectional study was conducted between January and July 2008. A stratified random sampling was done. 1150 adolescents in 12 to 18 year age group in grades 7 to 12 in 10 co-educational schools (government run and private) were the subjects of the study. Behavioural and emotional problems were assessed using Youth Self-Report (2001) questionnaire. Family stressors were assessed using a pre-tested 23 item questionnaire. Univariate and multivariate analysis were performed. Multiple logistic regression analysis was also done.

 

Results

Prevalence of behavioural and emotional problems in adolescents was found to be 30%, with girls exceeding boys in all age groups. Internalizing syndrome was the most common (28.6%) psychiatric problem. On stepwise regression analysis, a perceived lack of emotional proximity to mother had the highest odds (3.489) followed by addiction in father (2.642) and marital discord in parents (1.402). Type of school, type of family, socioeconomic status, relationship with father, mother&s employment and educational status were not found to be significantly associated

Conclusion

An alarming number of our adolescents suffer from emotional and behavioural problems which have their roots in the family environment. These data suggest urgency in establishing a school based mental health service.

1. Why Puberty?

Why Puberty ?: Physically body of a child, be it a boy or a girl; goes through developmental stages. Every individual has different timetable of growth, which is normal. Puberty changes takes four to six years and transform child to adult; who becomes capable to reproduce. Few changes are common for boys and girls, while few changes are exclusively for Girls and few other changes are exclusively for Boys. Female alone or male alone can not reproduce. So the marriage takes place. After marriage, husband and wife to perform reproductive act together, which is called sexual intercourse. If wife conceives and becomes pregnant, a child is born after nine months or so.

Hormones: 
In Girls : The pituitary gland produces hormones that stimulate the growth of the ovaries. The ovaries then produce the hormone Estrogen and release it into the bloodstream. Increases in estrogen lead to breast development, changes in the shape and contour of the body, menstruation and maturation of the reproductive system. Estrogen sparks the release of the hormone called progesterone., which helps the thickening of the blood lining of the uterus. 

In Boys : The pituitary gland produces hormones that stimulate the testicles to produce the hormone testosterone. Increases in the level of testosterone lead to the growth of body & facial hair, the growth of the reproductive organs, the production of sperm, the changes in the shape of the body and the change in the voice. Other hormones, adrenal, affect the development of muscles and bones, the growth of hair, and the changes in the skin.

2 Body Odor

Body Odor : Sweat Glands are mainly responsible for perspiration. They are activated by temperature, exercise as well as emotional reactions. Fear, embarrassment, romantic feeling may also increase perspiration. A hormone called Adrenalin is released, which will result into more sweating. If you are the anxious or aggressive type of adolescent, you have to learn to relax and cool yourself. The oil-like secretions from these glands are the culprits behind acne and smelly scalps.

4.1 Breast Development : Breast will improve the looks and give psychological satisfaction. Due to the media exposure, well developed breasts is the dream of most of young adults. Although the size of the breast has nothing to do much with its primary function of breast feeding to child after delivery. Even the tiny breast is enough to produce milk for the child. 4.2 Bra selection: It is suggested to wear cotton bra only. If you wear close fitting clothes, then wear a seamless bra or bra with small dart stitching. Straps must be adjustable and have good elasticity. Rigid straps will cut into shoulders and if they are loose and springy they will not give adequate support to the breast. More details
How to wear the Bra : Girls may find difficult to close the hook initially, as it is normally on the backside. Discard the Old Bra : If you find uncomfortable even after adjusting straps and hooks level, you should change the bra. The straps elasticity as well as elastics provided near the hooks adjustment loose their elastic property, then discard the old bra and go for new one. Young girls may also have to change the bra due to the change in their breasts(CUP) size.
Bra & Cup size measurement:
Measure your rib cage under the breast in inches(M1) and add 4 or 5 inches to bring it to an even number(B1). B1 is the bra size always sold in even nos.; 28,30,32, 34, 36 etc. Measure(M2) across the top of your bust / nipple without a bra on. More details

4.5 Internal Anatomy of female : The reproductive system comprises of number of organs inside the body located in the lower ABDOMEN, safely under the pubic bone. Urinary opening under the inner labia, is connected to the URETHRA(tube that carries the urine from the BLADDER). The vagina, a tube-shaped spongy passageway, leads from the outside of the body to the muscular organ called UTERUS. The narrow lower end of the uterus is called the CERVIX, has a small opening that opens in the vagina. FALLOPIAN TUBES are connected to the right & left sides of the uterus. 4.6 The Process of Menstruation : Menstruation is triggered by a small gland located in the base of the brain, called the PITUITARY GLAND, which produces HORMONES (chemicals that act like messengers, traveling in the blood to the ovaries). Inside the body, the reproductive system begins to function. During each monthly cycle, one of the ovaries releases an egg cell (Ova), which is smaller than a grain of mustard. It enters the nearby fallopian tube and travels toward the uterus that has a special lining of blood and tissue, called the ENDOMETRIUM. More details..
 
4.7 Intercourse and fertilization (Girls) : Wife and husband have sexual foreplay like kissing, caressing, embracing, whispering, fondling etc. , which arouse sexual feelings. Finally during the act of sexual intercourse, husband penetrates his penis inside the vagina of her wife. The vagina also become moist and lubricated during foreplay. More details
4.8 Orgasm : It is also called reaching to climax. It is the ultimate feeling during sexual intercourse between husband & wife. After the fantasy and sexual foreplay, wife also get aroused. The caressing of clitoris and Labia with the hand as well as stimulation between penis and vagina makes the inner muscles of vagina to contract and result into sweet feeling of ecstasy. This may last for 5 to 10 seconds only. But that is the satisfaction every wife wants from her husband. Every time wife may or may not achieve the orgasm, while only the husband achieves the orgasm.

4.9 Masturbation (Girls) : Young adult girls do experience romantic feelings as well as fantasy. Before marriage and even after marriage, masturbation is the act of sex play which gives the feeling of Orgasm. But here the penis and its ejaculation of sperm in the vagina is absent. Instead, the stroking of clitoris and labia is carried out with hand or fingers, mainly by self or sometimes by husband. Masturbation is completely normal and many females have enjoyed, without the risk of unwanted pregnancy. Pics

4.10 Safe days to avoid Fertilization : Young Adult girls often wonder exactly what are "Safe Days". Stimulated by hormones from the pituitary gland, the ovaries produce the hormone ESTROGEN, which stimulates the onset of menstruation. A second hormone, PROGESTERONE, is produced by the ovaries when ovulation occurs. From beginning of menstruation between 11 to 14 years(Menarche) to cessation of menstruation between 40 to 45 years(MENOPAUSE), the rhythmic patterns of the female cycle are controlled by these hormones. Each time a girl gets her period, a new cycle begins. The cycle usually lasts about 28 days, but the length of the cycle varies from 21 days to 40 days. 4.11 Latex Condom : Safer way to intercourse: It is a male barrier method. Condom fits on to the Penis tightly, similar to rubber glove fit onto the finger. It stops the sperm to travel into the vagina after ejaculation. More details...
4.12 Irregular Periods : If an Ova is fertilised by a male sperm, then the next menstruation period will be missed. It also means that a child is conceived and wife has become pregnant. The menstruation can only start after the delivery of a child or termination of the pregnancy(Abortion). If the regular period is delayed by more than 15 days, young adult girls should consult the Gynecologist to confirm the reasons. A simple urine test for pregnancy can be carried out to confirm the pregnancy. A change in environment or routine or any emotional stress or illness may also upset the cycle, causing the period to begin later or earlier than expected. 

4.13 Abortion : The ova is fertilised by sperm and pregnancy is then confirmed by the gynecologist. It must be a good news for newly married wife. But it may be bad news for unmarried adult girl or for the wife who wants to delay the arrival of child or for the wife who already have enough children. In such case, gynecologist will advise to go for early abortion of Fetus(unborn child), before it grows large and may require major surgery subsequently. 

4.14 Vaginal Secretions : Young adult girls may have sexual fantasy, which may induce the hormones to release sticky liquid in the vagina. (The same liquid helps to lubricate the vagina during intercourse.) The thick liquid may stain the crotch of panty, as it comes out after sometime. Girls may unnecessarily worry about the normal vaginal discharge /secretion. After menstruation, the discharge may appear as a sticky white or clear substance. Around the time of ovulation, it becomes more abundant and slippery in appearance. Vaginal secretion that is excessive or discolored, that has an unusually strong or bad odor, and/or that is accompanied by burning, itching, or irritation may indicate that a problem/ infection exists. She should tell see a gynecologist.

4.15 Personal Hygiene Teenage Girls : More careful attention to personal cleanliness during menstruation will contribute to a girl's comfort and confidence. Once menstruation begins, the production of oil and perspiration may increase during the days before the period is due. The following tips can be helpful as girls begin to develop personal care habits:
==> Girls should gently cleanse the external genitalia ( mons, labia & clitoris) with soft soap & water as part of their daily bathing to help eliminate odor. 
==> After each toilet use, girls should wash / wipe the anus from front to back to avoid the spread of bacteria from the anus to the vulva. 
==> Girls should wash their hands before and after using the bathroom and whenever changing menstrual cloth/pad. 
==> Panties should be changed daily. Cotton panties or panties with a cotton crotch are recommended because of cotton's superior absorbency. 
==> The menstrual stain should be avoided on dress / panty. If panty get stained during menstruation, presoaking in cold water will help loosen stains. Girls can also keep separate set of panties for menstruation days.
==> Girls should change sanitary pads as often as needed, to feel comfortable and to prevent odor from forming to high level. Odor develops when the menstrual flow leaves the body and comes in contact with air. 
==> Girls should wrap pads in toilet paper/old news paper & put in the wastebasket. 
==> Girls should carry at least one sanitary cloth/pad when they are expecting their period

I have experienced bleeding of teeth. This takes place at least once in a month. Can you please tell me if this is due to some inner problem? Sometimes I also feel gloomy and lonely. Most adolescents are pleased by the expected body changes that make them look more like an adult. At the same time, they are concerned about whether their bodies are "normal." A young girl may be convinced that something is wrong with her if her developmental stages are different from those of her friends. A girl who develops early may delight in being the first one who has to wear a bra or she may become self-conscious about her womanly body. Some girls worry because they have not begun to menstruate, while their classmates already have their periods. A young boy may be convinced that he has a medical problem if he doesn't have enough facial hair growth like his friends. A boy who develops early may be proud of his physical development or he may feel embarrassed because he looks so different.

1.2 Hormonal changes : Hormones are chemical substances, which affect growth and change in cell activity. Three physical organs involved in the production of the necessary hormones for pubertal development are the HYPOTHALAMUS, the PITUITARY GLAND, and the OVARIES in girls and TESTICLES in boys. The hypothalamus, a nerve center in the brain, releases chemicals into the bloodstream. The bloodstream carries the chemicals to the pituitary gland, an organ situated near the hypothalamus.

1.2A Hormones In girls : The pituitary gland produces hormones that stimulate the growth of the ovaries. The ovaries then produce the hormone estrogen and release it into the bloodstream. Estrogen is responsible for many of the changes that occur during puberty. Increases in estrogen lead to breast development, changes in the shape and contour of the body and the maturation of the reproductive system. Estrogen sparks the release of the hormone called progesterone., which helps the thickening of the blood lining of the uterus.
1.2B Hormones In boys : The pituitary gland produces hormones that stimulate the testicles to produce the hormone testosterone. Testosterone is responsible for most of the changes of male puberty. Increases in the level of testosterone lead to the growth of body & facial hair, the growth of the reproductive organs, the production of sperm, the changes in the shape of the body, and the change in the voice. 1.6 I PreMenstrual Discomfort (PMS) : Most girls never experience any cramps or discomfort, maybe just a few occasional twinges. These twinges are usually just muscular contractions of the uterus. They are felt in the lower abdomen on the first or second day of the menstrual cycle. Some girls may also experience headaches, depression, backaches, swollen breasts, a slight weight gain, acne, or sudden mood swings, etc. before/during period. Girls should note down such observations and speak to parent if their symptoms continues or become severe. Normally, cramps can be relieved by heat application (a warm bath or heating pad) or by medication.
1.6 J Problems in Menstruation : Occasionally, girls worry and need advice on unexpected changes in their menstruation cycle. A problem exist, if even after more than two years of menstruation, periods are consistently irregular and unpredictable. A menstrual pattern usually becomes regular & predictable within two years. Menstrual flow that is excessively heavy or long(lasting more than the individual's usual number of days), should be reported to a doctor. If menstruation stops for more than two months after regular period pattern is established, then consult gynecologist. About 95% of all girls have begun menstruating by age 16. Thus, a physical examination is advisable if menarche has not occurred by age 17. Any bleeding from the vaginal area that occurs between periods should be discussed with parent/doctor.
1.6 K External Protection : Many girls use cotton clothes to be kept between vaginal opening & panty. The old cloth is cut as per their requirement of sizes and changes once or twice a day depending on the flow. Such clothes are reused after proper washing with antiseptic and drying in the sunlight. A variety of ready to use normal or self-adhesive sanitary napkins that adhere to panties, are available. Many girls like the thin sanitary-pads because they are comfortable for active lifestyles. Girls can try initially different menstruation protection cloth/pad available and decide whichever is the most suitable to them. The shape of the face will change, shoulders will broaden, and body strength will increase. Voice will deepen as vocal cords grow. Cracking in voice can occur during this time of growth.
The pituitary gland produces hormones that stimulate the testicles to produce the hormone testosterone. Testosterone is responsible for most of the changes of male puberty. The growth of body & facial hair, the growth of the reproductive organs, the production of sperm, the changes in the shape of the body, and the change in the voice are result of increase in the level of testosterone. Other hormones, adrenal, affect the development of muscles and bones, the growth of hair, and the changes in the skin.
1.7 B Growth of Testes and Penis : The changes of puberty for boys usually begins with the enlargement of the testes/testicles. This development may start as early as 9 years of age, with the average age of about 12 years. Some boys may notice that the testicles have grown; others may not. At this time, the scrotal sac (Scrotum) loosens, grows longer, and deepens in color. During puberty, the penis will grow longer and wider. This growth may start as early as 10 years of age or as late as age 14, but the average age that growth begins seems to be at about 12 years. As the penis grows, its skin will become dark in color.

1.7BX : Teenage Boys Ejaculation :Usually between the ages of 11 and 15; boys may experience their first ejaculation, a landmark of achieving adulthood. 1.7 C External Anatomy : The two main parts of the external GENITALS, are the PENIS and the SCROTUM. The penis is made up of the shaft and the head or glans. The glans is covered by the foreskin, which is sometimes removed at infancy by a surgical procedure known as CIRCUMCISION. The small opening in the center of the glans through which urine and SEMEN are released, but one at a time. The scrotum is a loose sac of skin and muscle that lies under the penis. The scrotum houses two TESTICLES which produce sperm cells. The scrotum regulates the temperature of the testicles in order to make sperm.
1.7 D Internal Anatomy : The system is designed to produce, store, and transport sperm cells. The testicles / testes contain chambers / tubules, where sperm is produced. The testicles also produce the hormone TESTOSTERONE that causes most of the changes in males during puberty. 1.7 E Appearance of Pubic Hair : At puberty, boys will notice hairs at the base of the penis. During this developmental stage, pubic hair may gradually darken and may become more curly and coarse. When pubic hair begins to grow, boys may worry about small bumps that may appear at the base of the penis and on the scrotum. These bumps are normal. Boys will soon notice tiny hairs growing through the bumps. There may be other bumps consist of oil and perspiration. These bumps are also normal.
1.7 F Growth of Beard & Moustache : Another major change in appearance in boys will be, growth of Beard and Moustache. Initially few hairs will grow on upper lip and slowly it darkens. Then after, hair will grow on cheek & chin area. The area coverage will increase. But some boys may have very scanty hair growth even after they become adult. This is normal. The density of hair growth is different for individual boys, and so they should not worry about it. Even the time when it starts and reach full growth stage will also be different.
1.7 G Trimming or Shaving : Trimming of pubic hair can be done with a small scissors. The most important factor for a smooth, comfortable shave is using a sharp blade, to minimize drag against your skin. A blade should be replaced before it starts to irritate your skin, particularly in tender and more sensitive areas. Clean your razor / blade after every use, so that hairs, shaving cream and dead skin cells accumulated on the blade will be removed.
Using a shaving brush with a shaving soap/foam allows you to lather well. This gives hair more lift. Shave in the direction of hair growth, to reduce any damage to your skin. Using warm water on your hair prior to shaving gives the whiskers a chance to swell up a bit. This softens the whiskers and allows them to be removed more easily. Once you are finished shaving, rinse the area with either cold or lukewarm water and apply antiseptic after-shave lotion. One may get small, red bumps after a day, at few points of hair root. These bumps will disappear in two or three days.

  1.8 Socio Emotional Changes  
  Young adolescents begin to learn new social skills as they expand their social contacts. They also learn to cope with the excitement and confusion that accompany the changes. Adolescents begin to learn how to develop and maintain close, mutually supportive relationships with others of their own age. This social skill is essential for a normal, socially competent adult and is cultivated by involvement in a group.
Although adolescents approach to friends for companionship and for guidance in some aspects of behavior (dress, hairstyle, entertainment, etc.), they do approach their families for affection, identification, values and decision-making. Adolescents frequently seek the company of other adults, such as teachers, relatives or neighbors, who serve as role model or adviser. Adolescents want more autonomy in dress, outing, selection of friends, or how they spend their free time. They are not seeking complete independence but rather limited independence with a voice in determining specific rules and limitations concerning to them.
The selection of friends and dress, show their independent identity. They can discuss for hours on various issues. They feel that friends understand them better and have common concerns. They seek little privacy to create their own world, where parents are not allowed.

  1.9 Intellectual Changes
  The intellectual development, the ability to think abstractly, begins during early adolescence. Younger children think concretely, while young adolescents may think about "what might be" instead of "what is." They begin to develop the ability to understand science and mathematical concepts and to reason about ideals like justice, religion, or love. Young teens may question accepted rules and may argue whether rules are reasonable and fair. They may question others' beliefs and values.
The discussions and debate on many issues become stormy. They may challenge the belief system. Like, if God is everywhere, then why should they go to Church/Temple for prayers? They seek logical answers to many questions.
   1.10 Beware of Sexual Child Abuse

Sexual child abuse goes unnoticed and not complained about, as most of the times; the act is done by elders from neighbors, cousins, relatives, housemaids or friends. It may be in the form of talking of obscene language, showing pictures of obscene sexual act. Forcing to show innerwear as well as genitals. The act may be caressing of genital and other body parts, kissing and pressing, involving in the act of masturbation or upto the extreme level of physical intercourse. The child is scared to divulge the information to parents, due to family position and respect to elders. Some times child is under threat from the abuser. This is called Incest.
Incest is most common form of child / teen sexual abuse. Most of the time it is easy for them to be laone in room and house which tempts such act. The brother sisters, cousins do play together since their childhood. Their games involve physical touching and holding, which were very innocent way to play during childhood. But as they grow and few of them start getting sexual feelings due to reproductive hormonal changes in their body, those innocent games may slowly result into sexual fantacy and sexual pleasure. Sister and brother sharing same bedroom, coat and even changing clothes in front of each other during childhood may continue during puberty. Sometimes such intimacy result into Incest, where both of them enjoy each other body touch sexually. It is mupto parents to give separate place to sleep during and after puberty.
If the abuser is not a family relative, then the above act is called Molest. Here most of the time it may be elder family friend or neighbour or housemaid.
Some times if the victim is teenager then he/she may like the act due to the satisfaction of sexual desire. Teenage sexual abuse may result into repetitive act. As heterogeneous sexual act is difficult, such act may be between two male members as well as agreeable to both boy/male members, then it is called Homosexuality. If such sexual act is between two girls/females and also agreeable by both female members, then it is called Lesbian activity. Such GAY activities is increasing due to sexually active teenagers and young adults. Their biological need for sexual satisfaction may not wait until after marriage and it becomes difficult to observe abstinence.
Once the child understands about reproductive healthcare, about various parts of reproductive system, then he or she can understand well about child sexual abuse and able to resist, protect or ask help from parents. Unless and until parents are friendly or the child/teenager have confidence in their parents, the child sexual abuse remain with the victim only.

  3.1 Conversations with Adolescents

Initiate Conversations with Adolescents : While we want our adolescents to feel comfortable enough to come to us with any questions and concerns. We would like them to give us the opportunity to begin conversations, but this doesn't always occur. That's why it's perfectly okay, at times even necessary for parents to begin the discussions.
TV and other media are great tools for this. For instance, that you and your 11 year old daughter are watching TV together and the serial or advertisement includes a menstruation or sanitary napkin. After the show is over, ask your daughter what she thought of the serial or advt.. Did she know about menstruation? Just one or two questions could help start a valuable discussion that comes from it.

  3.2 Abstinence Message

Nowadays, the onset of puberty is early by almost two years or so, while the marriage has been postponed by almost five years or so. Physically the body has grown to produce the child, but child birth is permitted after marriage in our society, which result into almost 10 to 12 years period period between physical ability and marriage. Adolescents should be physically, cognitively, and emotionally aware about mature sexual relationship and their consequences like AIDS, premature pregnancy and risk of abortion.
The key goal of comprehensive reproductive healthcare education is to help adolescents to postpone sexual intercourse until marriage. Interventions those are effective in encouraging teenagers to postpone sexual intercourse, help young adult to develop the interpersonal skills they need to resist premature sexual involvement.
A strong abstinence message as well as information about contraception and safer sex will make them responsible about themselves. For interventions to be most effective, young adult need to be exposed to the information preferably 18 to 20 years of age. Reproductive healthcare education does not encourage teens to start having sexual intercourse, increase the frequency of intercourse, or increase the number of sexual partners.

  3.3 HIV / AIDS

HIV/AIDS : What we and the adolescents we care about need to know 

HIV : Human Immunodeficiency Virus is that causes AIDS. A member of a group of viruses called retroviruses, HIV infects human cells and uses the energy and nutrients provided by those cells to grow and reproduce.
AIDS : Acquired Immuno Deficiency Syndrome is a disease in which the body's immune system breaks down and is unable to fight off certain infections, known as "opportunistic infections," and other illnesses that take advantage of a weakened immune system.
A person who is HIV-infected carries the virus in certain body fluids, including blood, semen, vaginal secretions, and breast milk. The virus can be transmitted only if such HIV-infected fluids enter the bloodstream of another person. This kind of direct entry can occur 
(1) through the linings of the vagina, rectum, mouth, and the opening at the tip of the penis; 
(2) through intravenous injection with a syringe; or 
(3) through a break in the skin, such as a cut or sore.
When a person is infected with HIV, the virus enters the body and lives and multiplies primarily in the white blood cells. These are the immune cells that normally protect us from disease. HIV infection is the progressive loss of a specific type of immune cell called T-helper or CD4 cells. As the virus grows, it damages or kills these and other cells, weakening the immune system and leaving the individual vulnerable to various opportunistic infections and other diseases.

  3.4 Vitamin D

Vitamin D: What is it? 
Vitamin D, calciferol, is a fat-soluble vitamin. It is found in food, but also can be made in your body after exposure to ultraviolet rays from the sun. The major biological function of vitamin D is to maintain normal blood levels of calcium and phosphorus. Vitamin D aids in the absorption of calcium, helping to form and maintain strong bones. It promotes bone mineralization in concert with a number of other vitamins, minerals, and hormones. Without vitamin D, bones can become thin, brittle, soft, or misshapen. Vitamin D prevents rickets in children and osteomalacia in adults, which are skeletal diseases that result in defects that weaken bones.
What are the sources of vitamin D?
Food sources : Fortified foods are the major dietary sources of vitamin D. Prior to the fortification of milk products, rickets (a bone disease seen in children) was a major public health problem. One cup of vitamin D fortified milk supplies about one-fourth of the estimated daily need for this vitamin for adults. Although milk is fortified with vitamin D, dairy products made from milk such as cheese, yogurt, and ice cream are generally not fortified with vitamin D.
Exposure to sunlight
Exposure to sunlight is an important source of vitamin D. Ultraviolet (UV) rays from sunlight trigger vitamin D synthesis in the skin. Season, latitude, time of day, cloud cover, smog, and sunscreens affect UV ray exposure. It is especially important for individuals with limited sun exposure to include good sources of vitamin D in their diet.
When can vitamin D deficiency occur?
A deficiency of vitamin D can occur when there is limited exposure to sunlight, when dietary intake of vitamin D is inadequate, when the kidney cannot convert vitamin D to its active form, or when someone cannot adequately absorb vitamin D from the gastrointestinal tract. In children, vitamin D deficiency causes rickets, which results in skeletal deformities. In adults, vitamin D deficiency can lead to osteomalacia, which results in muscular weakness in addition to weak bones.
Females who cover their body for religious/environmental reasons, and individuals working in occupations that prevent exposure to sunlight are at risk of a vitamin D deficiency. If these individuals are unable to meet their daily dietary need for vitamin D, they may need a supplement of vitamin D. Individuals who have reduced ability to absorb dietary fat (fat malabsorption) may need extra vitamin D because it is a fat soluble vitamin. 
Vitamin D and osteoporosis
Osteoporosis is a disease characterized by fragile bones. It results in increased risk of bone fractures. Having normal storage levels of vitamin D in your body helps keep bones strong and may help prevent osteoporosis in elderly, non-ambulatory individuals, in post-menopausal women, and in individuals on chronic steroid therapy. Normal bone is constantly being remodeled (broken down and rebuilt). During menopause, the balance between these two systems is upset, resulting in more bone being broken down than rebuilt. Estrogen replacement, which limits symptoms of menopause, can help slow down the development of osteoporosis by stimulating the activity of cells that rebuild bone. 

Vitamin D Deficiency:
Prolonged deficiency of vitamin D results in changes in the bones of children and adults, and possible hearing loss with aging. In addition, rickets (in children) and osteomalacia (in adults) where bones are malformed and weak from poor calcium and phosphorus deposition. Osteoporosis is another adult disease that results from a poor dietary vitamin D and calcium intake. Hearing loss from vitamin D deficiency may progress as the adult ages due to increased porosity of the cochlea bone in the inner ear. Vitamin D supplementation usually repairs conditions caused by poor dietary intake. Prolonged exposure to sunlight does not cause vitamin D toxicity.
The best known activity of vitamin D is its role in maintaining bone. It functions by increasing the uptake of calcium from the intestine through interaction with the parathyroid glands in controlling bone resorption and serum calcium levels. The skeleton is the body's reservoir of calcium and provides calcium through resorption of mineral when serum levels of this essential element drop. Vitamin D also increases reabsorption of phosphate by the kidney tubule, and may directly affect the osteoblast, the cell which forms bone.
The diet is very low in vitamin D, unless you're consuming fish liver. Fortified foods, primarily dairy, are our major sources and may be produced by irradiation or by addition of synthetic forms of the vitamin. Supplementation with fish liver oils or extracts of these oils in capsules are important sources, especially for children or seniors living at higher latitudes and particularly during fall and winter. A reduced incidence of hip fractures has been shown for seniors using calcium and vitamin D supplements.

These critical lessons in personal development can have positive effects on students' sense of self-image and help them to take their first confident steps toward adulthood with a minimum of emotional discomfort and embarrassment. In addition, to striking the right tone of expert guidance & friendly support with the students, it is important to make sure that parents have the information they need to help their youngsters through this exciting - but challenging - time of life.

Clear Communication : Workshop discussion rules are developed, so that all students will feel comfortable sharing thoughts and feelings. Be positive! Educator help adolescents to develop attitudes about human development by setting the tone of the learning experience. How something is presented is often more powerful than what is said. A climate of openness and trust is encouraged. Students' comments and questions are accepted with identity or without identity. Let them know that their concerns and opinions are valid and worthwhile. We provide opportunities for students to ask questions anonymously, which is the easiest way to express their concerns. WE give students time to write down questions and deposit them in a Question Box. Then after we answer the questions later for the entire class. We encourage students to use proper terms for the parts of the body and their functions. Avoid putting anyone on the spot. 

Parents Involvement : Encouraging parents' involvement when teaching about adolescence growth and development is important, because parents are the major influence on adolescents. Most parents support education on physical development in workshop. Discussions between a parent and a child can be difficult and awkward when they concern issues of puberty and human reproduction. Parents may feel that they lack sufficient information to answer their children's questions. Children may sense that their parents are reluctant to discuss these issues. Parents will welcome assistance in adjusting to the changes that their children will experience during early adolescence. Involving parents from the beginning is an important part of planning adolescence education. We invite parents to Parenting Workshop, where they will have the opportunity to:

"I think I'm NOT OK looking," the adolescent, age 12-14, felt that way. Some of it was due to the usual behaviour of children: comments on the playground, teasing by peers in the classroom, and so forth. Few adolescents may have felt: "Whenever I try to look at myself in the mirror and I think that my looks is not so good. "
There's a lot more to beauty than shapely figures, bulging biceps and blue eyes. Adolescents do often places too much emphasis on their appearance and how they compare with others. It would be shortsighted to try to convince our teenagers that they are good-looking. Teens in particular, often get so caught up in their appearance that the mirror becomes the sole measure of their worth. Every imperfection assumes huge importance, often exaggerated beyond its reality.
Teenagers face overwhelming pressure from television, movies, advertisements and their peers to believe that if guys don't look like Hritik Roshan or girls like Kareena Kapoor, they're unattractive and undesirable. In addition to this prepackaged image of what is beautiful and what is not, media also tout the idea that beauty equals happiness.
Overcoming such pressures is a formidable task, and one that requires patience and perseverance. No clever words or expensive purchase will overcome a teenager's feelings of inadequacy. We want our children to understand that there's a lot more to beauty than shapely figures, bulging biceps and blue eyes. We want to plant in them a conviction that beauty more often results from a person's attitude and disposition. Mahatama Gandhi is chosen Father of the nation, after all, not because of his appearance or physical attributes, but because of his other personality traits.
Too often we confine compliments to externals, such as a new outfit, rather than celebrating positive attitudes and positive behaviour about family relationships, life, school and people. So we adopted several practices that we think are helping our children develop appropriate, biblical ideas about their appearance and their value.

  3.7 Understanding our teenagers better! (parentips)

Can we observe our behaviour towards teenagers? :
When your teenage son or daughter comes home from the school / college today, receive with smile. Do that several days in a row and see the difference! Next time your teenager tells you a joke, laugh.
Make a list of all those behaviour your teenager have, that annoys you. Now, go through the list and delete all the points that doesn't really matter. Before you criticize your teenager's behavior, try remembering your own teenage years. Chances are it will help you communicate better. Save your anger only for those behaviour which have lasting moral consequences.
Go out for breakfast or lunch or dinner once a fortnight preferably at their choice of place. Promise yourself that you won't use that time to advice or lecture. Just listen and talk about good stuff. Experience the enjoyment together. Otherwise invite your teenager's friends to your house for snacks, soft drinks and a small get together. Arrange for the music of their liking.
Let your teenager keep his or her own money account for the month. Deposit monthly allowance and then allow him or her to do their own shopping for clothes, books, snacks, movies etc. and other necessities. This will help your teenager to learn the value for the money, responsibility and self-control.
You should allow your teenager's privacy. Snooping without a legitimate reason is not advisable. A college going teenager may search for their own identity and create a small world for themselves. Enter there with their permission.
You may ask your teenager to play his/her favorite music on your stereo. Listen and discuss the music with him or her. Find out why he/she likes it so much. Try to avoid criticism to their extra curricular activities and interests. Instead appreciate their efforts in achieving excellence in other fileds of life.
Love your spouse. A strong family provides security for teenagers. Mother and father happiness gives them more relief.

Branding : The research for an "effective name" becomes difficult, as most of the target audience would shy away from answering questions on personal issues related to reproductive healthcare. However, branding for personal hygiene products has been mainly on the product benefits and attitude.
* For instance, a name like "Stayfree" or "Carefree" clearly denotes the functionality of the product, thereby to a great extent leveraging the brand. Stay outside freely or remain carefree for complete day during menstruation.
* The females can identify with the name "Whisper" (most possibly because of the undertones in which sanitary napkins have always been spoken about in our country). It is sold wrapped in paper or black plastic bag and always kept hidden in the cupboard.

  4.2 Understanding Rural Marketing in India

 
There has been a vast difference between the rural markets and urban markets, within the rural areas also-- between regions, states and districts. There is a difference in the media reach due to the education levels, their culture and the type. This divide is expected to increase. The marketing strategies are entirely different, that is the reason many multinational companies failed to reach in villages. The Indian villages are highly heterogeneous in nature. That is why many marketing bigwigs sitting in metros are unable to understand the mindset of different villagers across Indian land.
The difference is in celebration of festivals, birthdays, occasions like marriage etc.. There is a vast difference in the lifestyles of the people in the two regions. The kind of choices of brands that a urban customer enjoys is different from the choices available to the rural counterparts. The difference is also in the way of their thinking. Their purchasing power and purchasing time are mainly agriculture related and get affected due to rain and seasons. The strategy revolves around what attracts the rural customer to a product.
For example - Packaging. The rural customers are usually daily wage earners and they don't have monthly incomes. So the packaging is in smaller units and lesser-priced packs that they can afford. The unit pack size has to be small. The shampoos in sachet pack became hit due to price to be paid whenever the rural females wash their hair. Then a thing like the colour choice that attracts is entirely different. Another important factor is convenience of use. Colgate came out with tooth powder packaging. First sachets were introduced to match their income level. Second- to cap this sachet for convenience of storage while use. One of the grey areas that needs attention is the retailer, who plays a very big role in rural sales. The rural customer goes to the same shop always to buy the things. And there is a very strong bonding in terms of trust between the two. The buying behaviour is also such that the customer doesn't ask for the things by brand. The retailer pushes whatever brand he wants to push as they can influence the buyer very easily and very strongly on the preferences. The retailer is a chief influencer in the buying decision. In the rural markets there are three sources of water - wells, handpumps and ponds. These becomes mass awareness points of purchase. Special stickers can be put on the handpumps, the walls of the wells. The idea is to advertise not only at the point of purchase but also at the time of consumption. The customer can also see the brand message when he was bathing or washing. The customers who purchase the brands get a sense of satisfaction by seeing their choice being advertised in the places while a question was put in the minds of the customers who had bought other brands. So this is one of the innovative strategy that may work quite well.
Therefore to understand the way the rural markets work -- we need to go to these markets and spend time there in understanding them. We live in surroundings where the things are completely different from what the rural customer experiences. And we can't understand, unless we go and spend time there. The target audience for any product or service marketing is made up of several groups. In order to adapt the approaches to each group, it is necessary to differentiate between these groups.
Vulnerable Groups and Target Groups : It is important to differentiate between the vulnerable and target groups for intervention. The vulnerable group may in fact be the same as a target group of a communication, but this may not be always true. For example a vulnerable group at risk for personal hygiene consists of adolescents between 11 to 15 years. The personal hygiene care, reducing this risk will not be directed at the adolescents, but rather at all those involved in the care of them. An effective communication strategy takes into account; activities at all these levels. Each target group, especially the primary one must then be divided into its "segments" a term often used in marketing. Within the target audience there are primary, secondary and tertiary target groups.
The primary target group is composed of persons whose behaviour/habits/attitude is to be modified. These persons could be the parents of adolescents. In that case the aim would be to modify the manner in which they communicate or care for them.
The secondary target group is comprised of people who will be used as intermediaries to get the message across to the first target group. They could be health workers or teachers. It all depends on what the communication networks are existing in the society.
The tertiary target group is made up of people who can facilitate the communication process and behaviour change. They include administrators or celebrities, but also those persons close to the parents or adolescents and the extended family.
This implies that the approach will vary for segments differing in terms of educational level, socio-economic status, etc. We need to differentiate between the mothers who constitute the target group and between the various segments of the target group mothers, which will each benefit from a specific strategy. The media and support materials used to reach mothers who are functionally illiterate and living in rural areas will differ from those directed at more educated mothers living in urban areas.
Example: The communication on personal hygiene, the target groups may be defined as follows:
VULNERABLE GROUP: Adolescents
TARGET POPULATION: persons caring for these children.
Primary target group: mothers of the adolescents. 
Segment A: illiterate mothers living in rural areas. 
Segment B: illiterate mothers living in urban areas. 
Segment C: literate mothers. Etc.
Secondary target group: social workers and teachers in school, journalists from the local radio station / print media /TV media.
Tertiary target group: administrative officers from the education department, fathers of the adolescents, celebrities.
We must not forget that target groups should be participants in the process of social communication and not mere receivers of information. A one-way communication system for personal hygiene education would defeat our aim for an effective approach. Also, the target groups must themselves play a role in transmission of messages to other groups and to the "promoters" of the intervention. Influential people in the community should also play an important part within the secondary and tertiary target groups. This group is a catalyst in social communication.

Bra selection:
It is suggested to buy cotton bra only. Laces should be avoided because it may cut & scratch, in tropical Indian climate. If you wear close fitting clothes, then wear a seamless bra or bra with small dart stitching. Straps must be adjustable and have good elasticity. Rigid straps will cut into shoulders and if they are loose and springy they will not give adequate support to breast. A fastening with two or three hooks is much more comfortable, adjustable and supportive for daily wear. All straps should have adjustable levels for the hooks to be tightened or loosened to accommodate fluctuation of weight or washing changes.
How to wear the Bra :
Females should wear…. Girls may find difficult to close the hook initially. One can take bra and wrap around below the rib cage and bring hook in front, so that you can close it easily. Generally 3 adjustments are given for hooks. You can try one by one to see which one suits you. Comfortably. Then after closed hook bra move over the rib cage, so that hooks go in the backside and bra-cups come in the front side. When to discard the Old Bra :
If you find uncomfortable even after adjusting straps and hooks level, you should change the bra. The straps elasticity as well as elastics provided near the hooks adjustment loose their elastic property, one should discard the old bra and go for new one. Young girls may have to change the bra due to the growth development of their breasts.
Females should wear bra, straight at a horizontal level from the front and at the back. Unless a bra fits just right, it could ruin the shape of breast. Never buy a bra without trying it on & never ask others to buy for you. Because nobody knows your body like you do. 

Bra & Cup size measurement:
Measure your rib cage under the breast in inches(M1) and add 4 or 5 inches to bring it to an even number(B1). B1 is the bra size always sold in even nos.; 28,30,32, etc. Measure(M2) across the top of your bust/nipple without a bra on. Then the cup size(C1) will be M2-B1 that means, subtract the bra size (B1) from this bust/nipple measurement(M2). If the M1 is 29-30 your bra size B1 is 34". If you measure M2 as 35", then (M2-B1=C1) will be 1" that means cup size is B. Corresponding to difference(M2-B1) the cup sizes are given as under :

  5.4 For Girls : My first Branian

Young girls wear frock petticoat under their frock skirt or shirt, as undergarment covering their upper body, while the Boys wear "Baniyan' under their shirts. The boys continue to wear Baniyan even after they reach adulthood and throughout their old age. But girls wear 'Bra' after their breasts grow fully, somewhere between or at the end of their puberty period during adolescence.
BRANIAN for girls is very similar to 'Baniyan' for boys, except that the length is short, which ends near the ribcage of the girls. Sanitary Napkins are used by girls during their menstruation. It is outer protection to keep the panty and dress protected from messy brown/red flow and its stains. Sanitary Napkins come in various lengths and thickness. Thin elastic belt is provided with every pack of sanitary napkins (with belt type). Instruction are provided for how to use the belt with sanitary napkins. Some females do not use belt, instead simply keep the napkins at the crotch of a tight fitting panty and wear them. Cotton is the best absorbent of menstruation blood flow without any side effects. Teenage girls can prepare their own cotton pad using the above steps 1-5. It is advisable to select and buy the new cloth instead of using old cloth of used up dress. Keep soft plastic strip in the fold for better security to avoid staining of panty. Place it in such a way that one fold of cloth cover should be between panty and plastic strip, while 3 folds of cotton cloth between vagina and plastic strip for more absorbency.
Menstruation. On that first day of period, the girl would never have imagined to see blood in place of urine slowly coming out of vagina. But then a lot of things didn't really turn out as expected. Also, girls do not have any control over it on timing, nor they can resist for few minutes. So, one need to keep extra protection to keep spare panty and dress from getting stained.
Simply keep the good cotton or terry cloth, which can be found easily in the house. Red or brown or such flower print would be a good colour. Simply fold in half, then half again - both lengthwise. You should have a long narrow pad now. Place in the crotch of the panty, and generally cloth against cloth doesn't move due to friction. For more absorbency, simply fold two cloths together or use organic cotton cloth. For waterproof protection, you can cut strip of a transparent plastic film, and lay it at the bottom layer of cloth pad. One can try out and arrive at the thickness and absorbency level which is comfortable.
Washing care : Wash your pads before their first ever use to increase absorbency and to remove any undesirable chemicals. 
After use, unfold the pad, soak them in water and rinse as you go for bath. If you soak them in cool water you will find staining to be minimal or nil. Then wash with detergent. Again soak in water mixed with antiseptic solution if desired. Take it along with the bath towel and hang to dry. Iron it after drying, if you want for better comfort during next use. After few uses, if the pad becomes hard and stained, replace the same with new one.

Acne is caused when the pore openings on the face, neck, back, chest and arms become plugged with too much oil production made particularly during adolescence when the hormones are elevated and the oil glands have increased in size. Shampooing: Regular shampooing is also recommended as oil from your hair can make acne worse particularly around your hairline. If your hair is oily, you may want to wash it more often.

  6.1 Understanding Body Image


Everyone has a body image. How do you describe yourself. It is basically the mental picture you have of your own physical appearance and the attitudes and feelings you have toward it. An unrealistic body image occurs when your mental picture is unrelated to physical reality. Many adolescents reject their body, no matter what their actual body size or shape. They despair about their height, breast or skin complexion rather than appreciating their individuality and viewing their physical attributes favorable.
Strong socio-cultural reasons to achieve the ideal body size and shape, which is unattainable for most adolescents. Undue emphasis is often placed on body image. Adolescents tend to disregard their successes in all other areas and have low self-esteem because their body is not in the ideal shape.
* Close your eyes, put your hand on your body part and say thanks to your body parts one by one : eyes, ears, nose, hands ….. Try a tender message for helping you to live the life in this world.

* Begin by accepting that it's what's inside that counts, and work at describing the importance of looks in your life.

* Spend few minutes daily looking at your body in a mirror - don't judge. Notice curves, length of arms, muscles, complexion etc. And Complement yourself.

  6.2 Take time for your Feet care

Feet are the most used parts of the body, but we rarely take time to give them the required attention. Pedicure and massage once in a week leave feet feeling refreshed and clean. Soak feet in a bowl of water for about ten minutes. Rub feet with a body scrub, so that any rough skin that may have formed on the heels and the balls of the feet can be removed. Wash off all traces of dead skin and dry the feet thoroughly by removing excess water. Wipe the feet with cotton towel. Cut the nails at the roots across and file smooth. If you keep nails longer, then remove dirt at the root. Cut them to give outer shape and file smooth.
Start massage with oil/lotion at the base of each toe/finger and work down the foot making small circular movements with your thumb and forefinger. Work the lotion between the toes and massage each toe by gently rolling it between the fingers. This helps to stimulate the circulation and pressure points. Lie down for about five minutes in relaxation position.

  6.3 Can Skin Glow?


Certain girls have a peaches and cream complexion while others have a dull and lifeless one. Where Does the Complexion's Healthy Glow Come From? Our complexion has its ups and downs, peaches and cream to dull and lifeless. Our complexion says a great deal about our physical and mental state. Emotions, nutritional excesses or deficiencies, fatigue, stress, age, all have an affect on our complexion.
Microcirculation is the deep-down source of a healthy glow. This effect is triggered by the mechanism of back scattering of light within the skin, making it glow with added freshness and a slightly rosy color. The skin's smooth surface allows for homogeneous light diffusion within the skin. If microcirculation has been effectively stimulated, light will more easily reflect off the red blood cells that become the source of the skin's color.
If we analyze one ray of light hitting the skin's surface, a small quantity of the light is reflected directly while the major portion penetrates the different layers of the skin. If the skin's surface is irregular, it absorbs light, breaks it up and reflects it in many different directions. The skin then has a lifeless, whitish appearance. This is particularly the case with dry, scaling skin. Greasy skin will also be reflected by excess sebum, giving skin a shiny appearance. On the other hand, smooth, clear skin will allow a better penetration of light. Reactivating microcirculation is sufficient to give the skin a Healthy Glow. In cases of extreme stress, fatigue or shock, the body blocks microcirculation in order to send more blood to vital organs. Stress provokes the contraction of micro-vessels. This inhibits blood circulation and the skin's rosy color fades. To maintain the skin's rosy hue, microcirculation must remain active. Be happy and tension free. A smile will make all the difference.

  6.4 Skin care of Back & Neck


There's a lot you can do for yourself . Rather than covering up and hiding your body parts, take care of the skin. During daily bath, one may not have enough time to pay attention on each and every parts of the body. A weekly light scrubbing down with pumice stone or a scrub gives a fresh and smooth look to the skin. The back, the shoulders and the neck area needs extra weekly care. There's nothing like jet shower massage to relax the tense nerves of the skin. A shower bath in the evening or before going to bath will also give relaxation as well as clean the skin from whole day's perspiration, body oil, dirt particles.

  6.5 Body hair care


Hair Dye : Hair coloring smoothen the outermost layer of hair. There appears to be no connection between the application of hair color and the appearance of gray hair. Hair turns grey due to the inability of hair to produce melanin in the early stages of hair growth. Melanin is the color pigment in your hair, which can be black, brown, yellow or red. The individuality of your hair color is determined by the percentage of each color pigment, making each person's hair color characteristically their own. Poor diet, illness, age, heredity, etc can cause the acceleration of grey hair and the actual use of a hair dye has nothing to do with the graying process. Hair can be checked out for porosity or elasticity, both of which play a major role in how healthy the hair will look after the color.
Side effects of hair coloring : If a professional taking all the precautions, the side effects are almost nil, especially if the one follows the after care recommended. However, it is very important that a patch test is necessary, with a new hair color. This is to check for allergic reaction before carrying out the coloring process. This should be done at least 24 hours in advance.
Frequency of hair coloring : Few colors fade easily and need application once a month, for example, to maintain the intensity. In the case of grey hair, it would depend on the individual growth rate of hair. On an average, hair grows ½"- ¾" every month, so a touch up once in 30 days is a must for grey hair. For those with no grey hair and a mild color not too different from their natural color, a touch up once in six months is ideal. For those with a drastic or dramatically different hair color, a root touch up is important. Too much exposure to sun, sea and chlorine should also be avoided.

Waxing : a thin layer of heated wax is applied on the skin in the direction of the hair growth of legs and arms. The hair becomes embedded in the wax as it cools & hardens. A strip of jeans type cloth is pressed on the above portion. The cloth is pulled off with wax & hair, quickly in the opposite direction of the hair growth, taking the uprooted hair with it.
Waxing can leave the skin sore and open to infection. Waxing should not be used over veins, moles, or warts. They should not be used on the eyelashes, inside the nose or ears, on the nipples or genital areas, or on irritated, chapped, sunburned, or cut skin. A small area should be tested for sensitivity or allergic reaction before treating the entire area. 
Hair will return within four to six weeks. If you don't mind pulling a sticky bandage off your own skin, and if you have too much unwanted body hair, then you may be the perfect candidate for waxing.
Bleaching : Bleaching does not actually remove the hair. It lightens the hair color so it appears blonde or golden. All hair bleach kits come with two essential components: the actual hair bleach mixture in cream form, and the activator mixture, which is always in powder form. Follow the instructions to obtain the proper proportion of bleach to activator. Never combine the two until you're ready to begin bleaching, always immediately discard any unused portions and never mix the two together in any kind of a metal bowl or container.
Whatever bleach you get, be sure to do a patch test. If no redness, irritation or eruption occurs, you can proceed with the full application. To date, no evidence has been presented of harmful effects of bleaching to the skin. However, the bleaching agents that lighten the hair can also alter the pigment in the skin. This can result in a change to the skin texture, making it thicker, coarser and sometimes darker.
Threading : It is mainly used to give shape to eyebrows and some facial hair on the upper lip. Threading utilizes a strong thread which is twisted and pulled along the area of unwanted hair, lifting hair directly from the follicle. The pain experienced during Threading is no worse than that experienced during waxing. A small pinch is felt with the removal of hair. Unlike waxing which can be messy and time consuming, Threading requires no external products, only the use of 100% cotton thread and a minimal amount of time with a skilled beautician. Threading does not have the unpleasant side effect of irritation and rashes caused by the "stripping off" of the top layer of dead skin that accompanies waxing. The benefits of
Trimming : During puberty initially most of the boys and girls like to cut/trim unwanted pubic hair and underarm hair. Due to physical activities and exercise lot of sweat comes out of the skin near hair roots mainly in the pubic area and underarm. Trimming of hair is essential to reduce sweat odour.

Shaving : It is the most common form of hair removal. Men have been shaving their beards & mustaches as well as under arms & pubic hairs for thousands of years. Now, many ladies routinely shave their legs & underarms and pubic hair. A clean razor with a sharp blade is essential for a safe and comfortable shave. Skin should never be shaved dry; wet hair is soft, pliable, and easier to cut. Contrary to what many believe, shaving does not change the texture, color, or rate of hair growth. It is quick and convenient, but can cause skin irritation, ingrown hairs and cuts from the razor blade.

* Self Shaving is carried out to remove unwanted hair.
* Pre-soak the area first. You can do this by taking a bath or a shower, or placing a hot washcloth over the area to be shaved for few minutes.
* Apply a shave cream with shaving brush. 
* Use a fresh blade safety razor. 
* Pull the skintight and shave in the direction of the hair growth. You won't get as smooth a shave as shaving in the opposite direction of the hair growth . But by shaving with the direction of hair growth may reduce those annoying red bumps that sometimes appear in a day or two. Never try to shave quickly, otherwise one may get cut unnecessarily.
* After shaving, dry your skin gently but thoroughly. Once the skin is completely dry, use antiseptic lotion.
After you've shaved a few times, you'll find that you can shave more often and with less itch and irritation. The itching will go away eventually, lessening each and every time you shave.
